Research question
Were there meaningful differences in two-year real-world outcomes between sirolimus- and paclitaxel-coated coronary balloon platforms?
Main finding
The numerical differences observed between the groups were not statistically significant.
About the study
Drug-coated balloons (DCBs) are used in selected coronary interventions to deliver an antiproliferative drug to the arterial wall during balloon inflation without leaving a permanent metallic scaffold behind.
This study, co-authored by Prof. Dr. Hakan Uçar, evaluated two-year real-world clinical outcomes following coronary DCB treatment and compared outcomes associated with sirolimus- and paclitaxel-coated balloon platforms.
What did the study investigate?
The study focused on two principal questions:
- What were the two-year clinical outcomes following coronary drug-coated balloon treatment?
- Were there observable differences between sirolimus-coated and paclitaxel-coated balloon platforms?
The primary endpoints were restenosis, target lesion revascularization (TLR) and major adverse cardiac events (MACE).
Study design
This was a single-center retrospective cohort study involving 92 consecutive patients who underwent DCB-based percutaneous coronary intervention at Istanbul Aydin University VM Medical Park Florya Hospital between January 2020 and December 2022.
Sixty patients were treated with a sirolimus-coated balloon and 32 with a paclitaxel-coated balloon. Outcomes were assessed over a 24-month follow-up period.
Main observations
At two years, the study reported:
Two-year outcomes
Two-year outcomes reported in the study
- 13.0%
- Restenosis
- 7.6%
- Target lesion revascularization
- 9.8%
- Major adverse cardiac events
These figures describe the outcomes observed in this specific retrospective cohort and should not be interpreted as general treatment success rates.
Patient-reported angina frequency and severity scores also decreased at the three-month follow-up.
Sirolimus versus paclitaxel-coated balloons
Numerically lower rates of restenosis and target lesion revascularization were observed in the sirolimus-coated balloon group.
Restenosis
Sirolimus: 11.7%
Paclitaxel: 15.6%
Target lesion revascularization
Sirolimus: 6.7%
Paclitaxel: 9.4%
The authors emphasize the need for larger prospective studies before definitive comparative conclusions can be made.
Why do lesion characteristics matter?
Clinical outcomes following DCB treatment may depend not only on the balloon platform but also on patient and lesion characteristics.
In this cohort, larger reference vessel diameter was associated with an increased risk of restenosis. The study discusses anatomical and procedural factors that may influence DCB outcomes, including vessel diameter, lesion length, calcification and bifurcation anatomy.
These observations reinforce the importance of lesion assessment and procedural planning when considering DCB-based coronary intervention.
Study limitations
The findings should be interpreted within the limitations of the study.
- It was retrospective.
- It was conducted at a single center.
- It involved a relatively small cohort.
- Device selection was operator dependent.
- Angiographic follow-up was clinically driven rather than systematically scheduled for every patient.
- Device-level differences in drug dose, excipient systems and coating technology were not systematically evaluated.
